History
~~~~~~~

- Health-Sickness Rating Scale (HSRS), Luborsky 1962 Arch Gen Psych;

- then Global Assessment Scale (GAS): Endicott et al. (1976), PMID 938196, Arch
  Gen Psych (= American Medical Association);

- then GAS modified to become GAF as Axis V of DSM-III-R (APA, 1987);

- remains part of DSM-IV-TR (APA, 2000);

- also modified GAF (mGAF), Hall (1995), Psychosomatics (= Elsevier).
